Job Description

 

Summary:

A Case Manager is professional work, which involves planning, coordinating and providing case management services for clients who present a wide range of psychosocial problems. The employee in this class is responsible for the development of comprehensive treatment plans and the provision of services with involvement from other staff to include planning, coordinating and providing case management services for clients who are often complex. The person who fills this position has a strong working knowledge of how social service systems operate and possess the empathy required to understand and relate to individuals who work within this field and the clients who interact with it.

 

Essential Functions and Responsibilities:

  • Contributes to the improvement of CaringWorks supportive service program. 
  • Conducts comprehensive person-centered assessments to determine the client’s needs and their capacity for individual living at admissions and throughout client’s enrollment as needed.
  • Encourage full client involvement and leads to the development of person-centered service plans with measurable goals and objectives to prepare the client for next steps in the program within the identified amount of time. 
  • Maintains documentation of client interaction via agency outcomes management database systems. 
  • Adheres to all client confidentiality requirements and standards.   
  • Coordinate and/or facilitate individual and group activities to address quality of life issues. 
  • Develops relationships with representatives in other agencies to support individuals attaining services such as additional mental health care, psychiatric medication, medical resources, financial assistance, legal advocacy, etc. 
  • Participate in weekly debriefing sessions, supervision, department, agency wide and other meetings, training and professional development opportunities as determined appropriate by supervisor.
  • Ability to build working relationships with special needs populations.
  • Apply self-help and support principles and techniques to problems and issues, communicate effectively with a wide range of individuals with different needs and objectives, demonstrate organizational and time management skills.
  • Effectively communicate, establish and maintain cooperative working relationships with a wide range of individuals and groups.  
  • Maintain cooperative, positive working relationships with residents and those serving the residents in their identified communities to remove barriers to housing.
  • Plans, coordinates and develops treatment programs for cases involving behavioral or social problems, emotional disturbances or physical disabilities.
  • Make appropriate referrals as needed.
  • Monitor on-going client needs during program and implement post program follow-up.
  • Assist in responding to difficult situations including issues of client self-care, client-client conflicts, etc.
  • Prepares and maintains accurate comprehensive case records on each client and/or families with current progress notes, individual treatment plans, case reviews, documentation and, where applicable, listing of referrals made.
  • Attend residential department meetings.
  • Assist clients with increasing independence and obtaining independent housing through a voucher program (if applicable).
  • Conduct monthly (or as needed) apartment inspections to ensure safety and cleanliness of the unit.
  • Build relationships and conduct outreach with agencies that serve the needs of the clients. Specifically target agencies that promotes the CaringWorks ASPIRE program-model.
  • This position is a mandated reporter.
  • Other duties as assigned.

 

Qualifications:

  • A bachelor’s degree in Social Work or related field from an accredited school is preferred, or a minimum of two (2) years mental health and/or substance abuse case management experience accompanied by high school diploma.
  • Minimum 1-3 years of experience in health/human services, nonprofit organizations, and/or homeless or vulnerable programs.
  • Experience working with homeless population(s) helpful.
  • An understanding of deaf culture and demonstrate cultural competency and knowledge of the issues relevant to persons with hearing loss is preferred.
  • Effective verbal and written communication skills.
  • Ability to use holistic, client-centered approach to recovery and mental health. Experience with Housing First model of care and/or Motivational Interviewing best practices preferred.
  • Must possess strong interpersonal skills in order to establish and maintain effective working relationships with clients, families, treatment team clients, and other personnel involved in the individual’s treatment. Ability to effectively resolve conflict and cope with crisis situations.
  • If applicable, must possess a valid GA state driver’s license and liability insurance minimums as outlined in Caring Works Liability insurance coverage.
  • Able to work evening and/or weekend hours for emergencies/crisis.
  • Strong team/consensus building skills.
  • Strong and timely documentation and assessment skills.
  • Must pass a GAPS (fingerprint) records check and a drug screen.

 

Required Skills:

  • Excellent interpersonal, written, and oral communication skills.
  • Ability to multi-task and manage and organize projects.
  • Efficiency in Microsoft Office software to include Word, Excel, Outlook.
  • Ability to work with grantors, funders and other collaborative agencies.
  • Ability to work independently and manage multiple priorities.

 

Preferred Skills:

  • Efficiency in Google Docs/Google Drive.
  • Ability to work with at-risk client populations (i.e. chronic illness, mental illness, substance abuse, etc.).
  • Experience with ClientTrack/HMIS.
  • Experience with Coordinated Entry and state-agency referral process.
  • Familiarity with current practices and treatment of identified mental health and substance abuse diagnoses.

 

Work Environment and Physical Demands

Some employees in certain positions may have direct contact with the general public and clients in office environments and/or work areas may pose risk exposure to irrational/hostile behavior, contagious diseases, or contact with service animals.  Working with and assisting clients with a variety of emotional and behavioral needs may be stressful.  Staff may occasionally travel to meet clients and patients in their home and/or respective place they may be residing.

 

This position operates in both a professional office environment and community (i.e. client’s home, 3rd party offices, etc.). The position may require sedentary daily tasks with some movement between standing, sitting, and driving. The position requires communication to include talking, hearing and seeing in an effort to relay and report information.  This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines and at times may require reaching, bending or lifting up to 25 pounds. 

 

The noise level in the work environment is typical of that of an office or an open community setting. Employee may encounter frequent interruptions throughout the workday.
